Jessica Bartel

Jessica Bartel

Health Center Administrator

Jessica Bartel oversees the experienced, dedicated team responsible for providing outstanding care at Radford Green Health Care and Rehabilitation. She enjoys creating meaningful experiences and making a difference in the lives of residents and staff. Together, Jessica and her staff create an inviting and engaging atmosphere that meets residents’ needs and ensures their comfort and safety.

Prior to joining Radford Green in 2022, Jessica spent more than 15 years helping to serve the needs of older adults, including as director of food services and clinical nutrition for local rehabilitation and nursing care centers.

In addition to being a licensed nursing home administrator since 2014, Jessica earned a master’s of science in nutrition and wellness and is a registered dietitian and licensed nutritionist certified in geriatric nutrition.

Nurse Liaison/Admissions Coordinator

Jessica is a registered nurse specializing in geriatric rehabilitation nursing since graduating from Marian University in 2009 with a bachelor of science in nursing (BSN) degree. Jessica joined the Radford Green at Sedgebrook team in 2012 as the nurse liaison in admissions. She works with hospitals and families, reviewing referrals, communicating needs and managing the admission process. Jessica strives to make the transition from home to hospital to Radford Green smooth and enjoyable.

Kim Akainyah

Kim Akainyah

Director of Nursing

Kim Akainyah has been part of the Sedgebrook clinical team since 2014. She has worked as the director of nursing for over 24 of her 36 years as a registered nurse. She is passionate about outcomes based on best practice standards, and has been instrumental in sustaining the highest level of nursing care wherever she has worked. Kim received her Bachelor of Nursing degree from Northwestern University and is committed to excellence in senior care.

Tiffany Zapata

Tiffany Zapata

Assisted Living Manager

Tiffany Zapata is a licensed practical nurse who specializes in dementia care and geriatric nursing. After serving in the U.S. Air Force, she completed a practical nursing program at Concorde Career College in Aurora, Colorado, in July 2007. Tiffany joined the Radford Green nursing team in December 2009, serving in many different roles within the nursing department. In April 2017, Tiffany advanced into the role of assisted living and memory care manager. This role includes working with residents, family members and staff to create individualized care plans; helping with transitioning into the assisted living setting; providing staff education; and overseeing daily functions of the assisted living department.

Monisha Jain

Monisha Jain

Director of Rehabilitation

Monisha Jain earned a bachelor’s degree from the University of Virginia and a master’s degree in physical therapy from Duke University. She worked in a hospital setting in northern Virginia, followed by home health and a sports medicine clinic in New York City that specialized in golf therapy. After several years of managing her own practice in India, Monisha returned to Chicago in 2003 to work as an outpatient therapy manager at Concentra for six years, while simultaneously earning her orthopedic specialization in physical therapy in 2009. Monisha currently manages home health, outpatient and skilled rehabilitation at Sedgebrook.
