As we mentioned before, Sedgebrook is a Life Plan Community, also known as a Continuing Care Retirement Community (CCRC). 

This means that when you join our community as an independent living resident, you’ll receive security for the future if your health needs change. Our on-campus wellness team performs health assessments to help you determine when another level of care, like assisted living memory care, skilled nursing, or home health care, is needed. If so, you’ll receive the care you need right here on campus.

What is assisted living memory care?

If our physicians and wellness team officially diagnose you or a loved one with memory loss, you’ll be eligible to move into our on-campus, certified assisting living memory care program at Radford Green Health Care and Rehabilitation.

This designated residence provides daily assistance, and around-the-clock individualized care from dementia specialists. 

Assisted living memory care in Lincolnshire at Radford Green includes:

  • Help with activities of daily living (ADLs) like bathing, dressing, feeding, toileting, medication management and mobility.
  • 24/7 nursing, internists, and Alzheimer’s care specialists
  • Private residential suites
  • Secure outdoor garden courtyard
  • Daily housekeeping and fitness programs
  • Cognitively stimulating activities
  • Full-service dining room

You don’t have to be an independent living resident at Sedgebrook to move to assisted living memory care. We also welcome outside admissions for those who have been diagnosed with memory loss.

Two major differences between independent living and assisted living memory care at Sedgebrook

  1. Location of medical care.
    For independent living residents, our wellness team can arrange in-home health services and assistance during periods of illness or injury recovery. For residents with more extensive needs, care is provided in our assisted living memory care Radford Green Health Care and Rehabilitation center.
  2. Style of residence.

In independent living, choose from any of the following floor plans:

  • One bedroom/one bathroom
  • One-bedroom/one and a half bathroom with den
  • Two bedroom/one bathroom
  • Two bedroom/two bathroom
  • Two bedroom/one and a half bathroom
  • Two bedroom/two bathroom with den

In assisted living memory care, choose private studio/one-bathroom or one-bedroom/one-bathroom floor plans.

Aside from how and where residents receive medical care and the floor plans available in each lifestyle, we encourage all residents to live as independently as possible.
