Our on-campus wellness team performs health assessments to help you determine when another level of care, like assisted living memory care, skilled nursing, or home health care, is needed. If so, you’ll receive the care you need right here on campus.

What is assisted living memory care?
If our physicians and wellness team officially diagnose you or a loved one with memory loss, you’ll be eligible to move into our on-campus, certified assisting living memory care program at Radford Green Health Care and Rehabilitation.
This designated residence provides daily assistance, and around-the-clock individualized care from dementia specialists.
Assisted living memory care in Lincolnshire at Radford Green includes:
- Help with activities of daily living (ADLs) like bathing, dressing, feeding, toileting, medication management and mobility.
- 24/7 nursing, internists, and Alzheimer’s care specialists
- Private residential suites
- Secure outdoor garden courtyard

- Full-service dining room
You don’t have to be an independent living resident at Sedgebrook to move to assisted living memory care. We also welcome outside admissions for those who have been diagnosed with memory loss.
